Warren House
Supportive Living, Inc. offers housing and professional care to survivors of brain injury at Warren House, which is located in Woburn, MA. With a home-like atmosphere and individualized support services, Warren House provides survivors with the ability to live more independent lives.
Residential features include:
- Five 2-bedroom apartments, shared by two individuals (each with his or her own bathroom), and five 1-bedroom apartments
- Barrier-free living space including a full kitchen, living room, and bathroom
- Heat
- Common areas including outdoor patio
Services include:
- Case management
- Managed/supervised personal care services
- Staff-assisted meal planning and preparation
- Independent living skills training
- Accessible transportation
- Cooperative agreements with local medical facilities, human service agencies, and community service organizations, to offer residents access to additional services based on their needs
Supportive Living, Inc. also offers eligible residents rental assistance through the US Housing and Urban Development Department (HUD), under the Section 811 Project Rental Assistance Contract (PRAC). With PRAC, residents pay 30% of their adjusted income toward rent and the balance is subsidized by HUD.
