Support Services – Application Process
SLI has partnered with Advocates since 2007, providing the highest quality of health care services for residents, including:
Tailored support systems to meet individual’s needs and wishes
Nursing support
Case management
Managed/supervised personal care services
24-hour, seven days a week staffing
Staff-assisted meal planning and preparation
Independent living skills training
Accessible transportation
Cooperative agreements with local medical facilities, human service agencies, and community service organizations to offer residents access to additional services based on their needs.
Once SLI has pre-qualified the applicant for affordable housing staff will set up a zoom interview with the Advocates team, the applicant, and any support team that will be helpful in identifying what kind of care is needed. This initial meeting helps us to determine if the applicant would qualify for services through Mass Health. If the applicant meets the criteria they will be placed on our waitlist.
When a unit becomes available the next person on the waitlist would be contacted and Advocates would schedule an in-home visit with Nursing and OT to complete an application for services to MassHealth. Once the application for services has been approved the applicant is eligible to move into our community.
